The DMV Completion Card 30 day temporary license/waiver expires 1 year from the date of issue.
Request for replacement of completion cards - by mail ONLY, include first, middle and last name, address, driver's license # and state the license is issued, DOB , phone number, class date along with payment of $35.00 per card for processing.
Please specify which card you are requesting to replace!

The MSF Basic RiderCourse card can ONLY be replaced 48 months or less from the date of issue. The DMV Completion card can only be replaced 12 months or less from original date issued.
